  Direct Primary Care is a financial practice model that can help reduce the cost of primary care.  While most individuals have health insurance, direct primary care can be a way to help save you some money on your healthcare costs.  At Heroes Healthcare Primary Care, we offer a Direct Primary Care Membership plan that DOES NOT utilize your health insurance for your primary care.  Instead, for an affordable monthly fee you can get most of your primary care office visits and in-house testing and procedures covered. 

 A signed membership  agreement is not an insurance plan, and not a substitute for health  insurance or other health plan coverage (such as membership in an HMO or PPO)  and does not satisfy any federal mandates . 

• 10 to 19 years old - $44 monthly
• 20 to 29 years old - $54 monthly
• 30 to 39 years old - $64 monthly
• 40 to 49 years old - $74 monthly
• 50 to 60 years old - $84 monthly

10% discount if paying for 12-month membership in advance.

If you have insurance, you are still eligible to use your insurance for services such as prescriptions, x-rays, outside lab services, and non-affiliated healthcare providers (orthopedics, urgent care, etc).
